If it sounds like our program is right for you, here's how to get started:

• Schedule and attend a free Medical Weight Management Program information session. It's a good idea to come to the information session knowing your schedule and when you will be able to make appointments. View our upcoming events ».
• Fill out a questionnaire. Before accepting you into the program, we need some information from you to help increase your likelihood of success. We will provide you with a comprehensive questionnaire at the information session to help us assess how best to help you in your goal to manage your weight. Feel free to complete the questionnaire in advance and bring it with you to your orientation session. Download questionnaire ».
• Read through the information booklet. You will receive the information booklet when you attend one of the in-person orientation sessions.
• Schedule and complete a physical evaluation and body assessment appointment. To become active in the Kaiser Permanente Medical Weight Management program, you must have a current physical evaluation and body assessment. If you'd like, you can schedule one at the information session. Scheduling an evaluation at the session will ensure you a more prompt entry into the program, if it is right for you.

Please note: You must complete your physical evaluation and body assessment before you will be able to begin your program.

After completing these steps, you will be enrolled in the Medical Weight Management Program if you qualify and have no health conditions that would prevent you from participating fully.
